Omnicare consultant pharmacists process admission medication regimen reviews (aMRR) as close to admission as possible, as well as reviewing their entire available medical record for any potential issues and insights. By identifying and communicating any drug-related, clinically significant issues, the aMRR optimizes the quality, safety, regulatory and overall cost of each resident’s medication regimen.

Omnicare consultant pharmacists utilize the CDC STEADI Checklist of Falling Assessment to identify moderate to high fall risk residents and perform subsequent fall risk reviews. Part of our risk assessment includes reviewing resident medications for newly admitted residents, existing residents and on-demand medication reviews post-fall as requested.

Antibiotic Stewardship and Antipsychotic Use Support

Our highly skilled clinicians work with our proprietary software, OSC2OR, to adeptly identify overuse, misuse and underuse of antibiotics and antipsychotics for increased safety and improved resident outcomes.
